To Register:
1.     Electronic camp registration forms and detailed instructions are provided in this section.
2. Open and complete one Youth or Adult Registration Form for each individual you wish to register for Day Camp. Save to your computer or other desired digital device (CD, server, memory stick, etc.).
3. Open and complete one BSA Health and Medical Form for each individual you wish to register for Day Camp. Only Parts A & C are required for Day Camp.
4. Open and complete one Family Tally Sheet for your family.
5. Submit completed Youth and Adult Registration Forms electronically by e-mail to npdaycamp.registrar@gmail.com or, if preferred, by delivery of disk or other useable memory device.
6. Do not submit the BSA Health & Medical Form electronically.
7. Print out, sign and submit paper copies of your Family Registration in a packet to your Pack Coordinator in the following order: your check or money order made payable to your Pack, Family Tally Sheet, Adult Registrations, Youth Volunteer Registrations, Cub Scout Registrations, Junior Camper Registrations, and the BSA Health & Medical Forms. Health & Medical Forms may be submitted in a labeled, sealed envelope for privacy.


Registration Forms
For 2009, the NPD Day Camp is using Adobe Acrobat forms to improve the registration process and security. You must use Adobe Acrobat Reader version 8 or later to open and complete the forms. If you do not have Adobe Reader version 8 or later, you can download it free from Adobe. You cannot use other PDF readers to fill in the forms as they are not compatible with the Adobe forms.

Parents Registration Information
Process: All family registration documents including registration forms for any and all cub scouts, youth volunteers, adult volunteers and junior campers shall be filled in, printed, signed and assembled into a packet along with a Family Tally Sheet and check or money order made payable to your Pack and submitted to the Pack Coordinator.
Each family shall also submit completed Youth and Adult Registration Forms electronically by e-mail to npdaycamp.registrar@gmail.com or, if preferred, by delivery of CD or other useable digital device. (Do not send Health & Medical Forms electronically.)

The Pack Coordinator will collect the family packets, assemble the Pack family registration sheets and submit them along with a Pack Coordinator Checklist and Pack payment to the Registrar on or before April 18, 2009.

How do I register my son for camp, or my Youth Volunteer to assist at Day Camp?
Step 1. Open the above Youth Registration Form and begin filling in the form for one camper. At the top of the form, select Cub Scout, Youth Volunteer or Junior Camper for the child you are registering. After completing the form, save the form to your computer or other desired digital device. If you are registering several persons, just save each form under a different name (example: Billys Camp Registration Form).
Step 2. To submit the completed form electronically, you can click on the SUBMIT button when the form is opened in Adobe Acrobat, or you can send the file as an attachment with an e-mail addressed to: npdaycamp.registrar@gmail.com
Step 3. Print all completed Registration forms, sign and date.
Step 4. Download and complete Parts A and C of the BSA Health and Medical Form, sign and date. ONLY paper copies of the Health & Medical Form are requested. For privacy, the form may be submitted in a sealed envelope labeled with the Registrant’s name and Pack number to maintain confidentiality.
Step 5. To register additional youth, open another Youth Registration Form and follow directions for the above Steps 1 through 4.
Step 6. After completing Registration forms for each registrant, open and complete the Family Tally Sheet. SUBMIT YOUR FAMILY REGISTRATION IN A PACKET TO YOUR PACK COORDINATOR IN THE FOLLOWING ORDER: check or money order payable to your Pack, Family Tally Sheet, Adult Registrations, Youth Volunteer Registrations, Cub Scout Registrations, Junior Camper Registrations and BSA Health & Medical Forms.

How do I register adults as Adult Volunteers at Day Camp?
Follow the above Steps 1 through 6 except utilize the above Adult Volunteer Registration form instead of the Youth Registration form.

Pack Coordinator Information
A Pack Coordinator must be identified and registered with the Day Camp Staff. To register a Pack Coordinator, please complete the Pack Coordinator Registration Form (2009_Pack_Coordinator_Registration.pdf), sign and mail form to the address indicated. Also, email the following information to the registrar: Pack Number, PC’s Name, Phone Number, E-mail Address and best way to be contact.

Unit Registration (Pack Coordinator Only): The Pack Coordinator shall collect all the registration and family tally forms for each family participating in Day Camp along with a check made payable to the Pack for the correct amount shown on the Family Tally form. Please have each family’s registration packet assembled in the following order: Family Tally, Adult Registrations, Youth Volunteer Registrations, Cub Scout Registrations, Junior Camper Registrations and BSA Health & Medical Forms.

Pack Coordinator please complete the Pack Checklist form and assemble the Unit’s registration packet in the following order: Unit Check, Pack Checklist, and Family Registration Packets (in same order as Pack Checklist). You can drop off the Unit’s registration packet to either of the following addresses:

Note: Both a hard copy and electronic copy of the Youth and Adult Registration Forms need to be received by the NP Day Camp Registrar in order for successful registration. Failure to submit either hard copy or electronic copy will cause the entire unit not to be registered. (Do not submit electronic copies of the Health & Medical Forms).

Completing the Adobe Acrobat Forms:
You must use Adobe Acrobat Reader version 8 or later to open and complete the forms.

To complete a form, just type or select fields highlighted in blue or red. Sections highlighted in red are required fields. On the Family Tally form, some calculations are computed to help you calculate the registration cost. Sometimes these calculations are not computed until your selections are fully entered. To verify that the calculations are all complete, please select the calculation button at the bottom of the sheet. After completing form, save the form to your computer (please rename the form). If you are registering multiple people just open and save each form under a different name (e.g. 2009_Youth_Form_MyName.pdf)
